Completed applications should be returned to the Starfish Initiative, 814 N. Delaware Street, Indianapolis, IN, 46204. Applications are reviewed on a first-come first-served basis.

In order to be eligible, you must:
- Believe intensely in your ability to graduate from college and fulfill your life dreams and purpose.
- Be enrolled in the 8th grade when you make application.
- Be accepted as a Twenty-first Century Scholar www.scholars.in.gov.
